About this policy

This policy describes information collected through the PrimeMD Clinics public website. The clinic’s Notice of Privacy Practices, effective October 1, 2026, explains how it handles patient health information.

Online inquiry submissions are temporarily unavailable. You can call the clinic or choose the email link to request opening updates. Please do not include medical details in general email.

Information we collect

• Before online submissions were paused, the interest form collected your name, email address, phone number, ZIP code, selected care option, preferred contact method, and acknowledgment that we may contact you. Earlier versions also asked for a program of interest. Those earlier submissions were stored with the website hosting service, rather than sent directly to a PrimeMD Google Workspace spreadsheet.
• If you choose the opening-updates email link, your email app opens a message to PrimeMD’s front office. Your message is sent through your email provider and received in PrimeMD’s email account only when you send it.
• When you visit the site: technical information such as your IP address, browser and device information, pages requested, and security or error records may be processed by the website’s hosting and security services.
• If you choose to load the embedded map: your browser connects to Google Maps, which may receive technical information and use its own cookies.

How we use information

Previously submitted form details and emails you send may be used to respond to your inquiry, contact you about PrimeMD and appointment availability, and manage prospective patient requests. We use opening-update requests to send the updates you ask for; tell us if you no longer want them. Technical information supports site delivery, troubleshooting, and security.

Who may receive information

Website hosting and technical service providers process information needed to operate and protect this site and, previously, its interest form. Staff and service providers assisting PrimeMD with inquiries may access earlier submissions for that purpose. This website does not send inquiries to Google Sheets. If you choose the email link and send a message, PrimeMD’s email provider processes that message. Google Maps receives browser information if you choose to load the embedded map or follow a Google Maps link.

This website does not use advertising pixels or third-party analytics scripts and does not send interest form submissions to advertising platforms. It does not use website visitors’ information for sale or cross-context behavioral advertising.

How long we keep information

We keep inquiry records while needed to respond to prospective patients and manage related practice communications, subject to applicable recordkeeping obligations. Technical records may be retained by hosting and security providers under their operational schedules. You may ask us to review or delete an inquiry record using the contact details below; some records may need to be retained where law permits or requires it.

Your choices

Online submissions are paused; call the clinic or choose to email us. To ask what information you previously submitted, request a correction or deletion, or stop opening updates, call or write to PrimeMD using the contact information below. We may need to verify your identity before acting on a request.

Browser privacy signals

Some browsers send Do Not Track or Global Privacy Control signals. This site does not use cross-site advertising tracking or sell or share visitor information for that purpose, so those signals do not change which optional advertising technologies load here. If our practices change, we will update this notice and the relevant controls first.
